The Care Manager plays a vital role within the interdisciplinary team, responsible for assessing, planning, advocating, and coordinating patient care from the moment of admission through to discharge. Position Overview: The RN Care Manager II - IPCM is integral to the interdisciplinary team, focusing on assessing patients' health requirements, crafting personalized care plans, and coordinating services to achieve optimal health outcomes. Strong communication skills are essential for engaging with patients, families, and healthcare professionals. This role also involves advocating for patients' needs, promoting health education, and ensuring smooth transitions across various healthcare settings. Clinical expertise, critical thinking, and compassion are crucial for enhancing patient well-being and improving healthcare results. Qualifications: Required Education: Graduation from an accredited School of Nursing Bachelor's Degree in Nursing is mandatory Required Experience: Minimum of 1 year in an acute care hospital environment Required Licensure: Active RN License in the state of employment Required Certifications: BLS (Basic Life Support) certification upon hire or within 90 days Preferred Certification: Certification in Case Management Preferred Experience: 2 years in an acute care hospital setting Experience in Care Management or Utilization Management Why Join Us?
